Job Summary
The Social Worker plays a key role in supporting People Living with HIV (PLHIV) by providing psychosocial care, promoting adherence, coordinating community follow-up, and linking clients to essential health and social services. Under the AHF-Rwanda program, the Social Worker ensures clients receive comprehensive, compassionate, and continuous support throughout their care journey.
Key Responsibilities
1. Psychosocial Support to PLHIV
- Offer individual and group counseling to newly diagnosed clients, adolescents, pregnant women, and others living with HIV.
- Address emotional needs related to stigma, disclosure, and mental health challenges.
- Support clients in developing coping strategies and building resilience.
2. Linkage to Care & Retention
- Ensure immediate linkage to ART initiation for newly diagnosed clients following national and AHF protocols.
- Follow up on clients who miss appointments or are lost to follow-up (LTFU) using appropriate tools and registers.
- Collaborate with clinical teams to monitor retention in care.
3. Client Education & Adherence Support
- Educate clients about adherence, treatment literacy, disclosure, prevention, and positive living.
- Support the formation and facilitation of peer support groups, including mentor mothers, expert clients, and adolescent clubs.
- Work with clinicians to identify and support clients struggling with adherence.
4. Home Visits and Community Follow-up
- Conduct home visits for defaulter tracing, psychosocial assessment, and family counseling.
- Work with Community Health Workers (CHWs) to ensure continued follow-up for vulnerable or hard-to-reach clients.
5. Case Management for Vulnerable Populations
- Identify and assess the needs of orphans and vulnerable children (OVC), adolescents, and socio-economically challenged clients.
- Link clients to available social protection services, including food support, school assistance, and income-generating programs.
6. Stigma and Discrimination Reduction
- Organize sensitization sessions in communities and schools to reduce HIV-related stigma.
- Provide guidance to clients on safe disclosure and how to handle discrimination in familial or community settings.
7. Coordination and Documentation
- Collaborate with clinical teams, data managers, and AHF focal staff to ensure holistic care delivery.
- Maintain accurate, confidential documentation of all social work activities and interventions.
- Submit timely monthly reports and case summaries to the health center and AHF site leadership.
8. Participation in Program Activities
- Attend and contribute to multidisciplinary team meetings, outreach programs, and quality improvement initiatives.
- Support AHF initiatives such as ART awareness campaigns, adolescent empowerment activities, and positive living programs.
Qualifications and Competencies
- Bachelor’s Degree or Diploma in Social Work, Counseling, Psychology, or a related social science field.
- At least 2 years of experience in a clinical or community health setting, preferably supporting HIV/AIDS programs.
- Training in HIV counseling, adherence support, or mental health and psychosocial support (MHPSS) is an added advantage.
- Strong interpersonal and communication skills.
- Ability to work with diverse populations including adolescents, vulnerable groups, and community leaders.
- High level of confidentiality, integrity, and empathy.
- Fluency in local language(s) and working knowledge of English or French.

This position supports evidence-based decision-making, monitors key performance indicators, and contributes to service delivery improvement through robust data management and analysis. Key Responsibilities 1. Health Data Collection & Validation Ensure complete and accurate data collection from all service points (e.g., OPD, ANC, maternity, HIV, immunization, family planning). Utilize national tools such as HMIS, SISCom, EMR, and official registers. Perform routine data validation by identifying and correcting errors, duplications, and missing data in collaboration with clinical staff. 2. Data Entry and Management Enter data into digital platforms such as HMIS, DHIS2, and EMR. Maintain updated and organized digital and physical data records. Secure data backup and enforce data storage protocols. 3. Data Quality Assurance (DQA) Conduct regular data verification, consistency checks, and triangulation. Lead or support DQA activities as per Ministry of Health or partner standards. Build staff capacity in data quality improvement. 4. Data Analysis and Interpretation Analyze data to identify trends and create visual dashboards. Provide data insights to guide management decisions and improve performance.
